Job Description

Position: Intake & Referral Specialist

Supervisor: New Hope Program Director

Division: Health and Human Services Administration

Department: New Hope Healing Grounds

Responsibilities

Responsible for conducting intake interviews and completing documentation for program participants; documenting program participants’ completion plan, court recommendation forms; and other required documentation. This position is also responsible for providing program participants with appropriate referrals and information regarding program services, community services, and conducting outreach, aftercare, outings, follow-up activities and this position on call after hours as assigned by supervisor and required by program.

Essential Duties and Responsibilities
  • Conducts interviews with program applicants and completes appropriate intake forms.
  • Conducts program participant orientation, including but not limited to program philosophy, rules, and schedules.
  • Makes referrals to counselors for individuals requiring additional medical or mental health services for clinic referrals.
  • Attends staff meetings and works closely to communicate and facilitate program participant concerns to counselors
  • Maintains accurate and updated outpatient files
  • Ability to provide updated information and reports to the courts, Program Director, or Counselors.
  • Tracks and maintains classroom/group attendance records for all outpatients
  • Provides empowerment and motivation for program participants regarding their program goals.
  • Ability to works independently with strong sense of focus, task oriented, and nonjudgmental attitude
  • Strong communication skills required to establish rapport with designated community resource representatives and in making appropriate referrals
  • Required to effectively communicate and maintain personal boundaries
  • Ability to provide crisis intervention
  • Maintains up to date knowledge of new information/developments in the substance abuse field
  • Completes closure paperwork and ensures participants’ files are completed prior to closing cases
  • Makes two monthly follow-up contacts for documentation of outcome evaluations.
  • Attends court hearings and provides the court with attendance/progress reports as requested
  • Assist with promotions and communication of New Hope program services at community events
  • Ability to facilitate or co-facilitate educational groups when needed.
  • Maintains a professional attitude and works in an ethical manner in all aspects of the job.
  • Transport clients to treatment centers and travel to treatment centers to follow up when clients are been admitted.
  • Must be on call as required and assigned in case 24/7 for transporting Tribal members to rehabilitation center or any other unexpected emergencies.
  • Will be required to work with clients for once-a-month Saturday outing on as assigned by the program.
  • Perform other work-related duties as assigned by immediate supervisor.
Education/experience requirements

Minimum Qualifications: Must have a High School Diploma or GED. And or equivalent work experience.

Must be able to comply with strict adherence to Federal Confidentiality Regulations and maintain program participants’ confidentiality.

Preferred Qualifications

Bachelor’s degree in social science or Related field including but not limited to:
Sociology, Psychology, Social Work, Public Health, Community Health, Health, Education. Working in a similar setting and relevant case management experience.
