The Social Worker will collaborate with residents and their responsible parties to oversee admissions, identify needs, provide referrals to community resources, and maintain the mental and social health of each resident.

• Develop relationships with residents and responsible parties for admissions, counseling, and needs identification.
• Facilitate social interaction, reality orientation, and intellectual stimulation for residents.
• Assist the Business Office Manager with financial needs and promote resident emotional security.
• Complete acute care plans and attend care plan meetings.
• Assist the Administrator with resident, family, and responsible party concern management.
• Attend committee meetings as required.
• Complete monthly QA reports.
• Refer residents for vision, dental, and hearing services as needed.
• Assist with room changes by contacting parties and completing necessary forms.
• Conduct initial, routine, and as needed psychosocial and socialization assessments; refer for additional services if necessary.
• Discuss advance care planning with responsible parties upon admission and complete required documents.
• Make morning rounds prior to morning meeting.
• Provide staff updates at the departmental head's daily morning meeting.
• Perform suicidal/homicidal assessments and required follow-up.
• Assist in investigations requested by the Administrator.
• Assist with the completion of the MDS as needed.
• Work rotating weekend manager on duty as scheduled.

• Bachelor’s or Master’s in Social Work.
• Licensed Social Worker in the state of Texas.
• Previous experience working in a skilled nursing facility is preferred.

The role requires occasional lifting, carrying, and moving of residents or equipment, pushing/pulling, sliding/transferring up to 50 lbs, and the ability to drive. Reasonable accommodations may be made for individuals with disabilities.
