Case Manager; Rn​/Lic

Responsibilities

• Coordinate with other departments to provide maximum level of services and demonstrate the knowledge and skill necessary to provide care based on physical, psychosocial, educational, and safety criteria appropriate to the age of the patients served in the assigned service area.
• Provide administrative support and entry of CMS-required patient data information into systems such as CMS, UDS, SMS, and CERNER, and enter daily patient charges to ensure fiscal compliance.
• Demonstrate accountability for and contribute to overall unit quality improvement.
• Assist unit staff with patient transport, transfers, and translations.
• Other duties as assigned.

• Minimum of two years clinical experience in a hospital setting for case management; preferred background in utilization review or discharge planning.
• Maintain current professional Texas RN licensure.
• Ability to work with multidisciplinary staff.
• Working knowledge of current Medicaid and Medicare guidelines.
• Graduate from an accredited School of Registered Nursing.
