Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Join as a traveling Physical Therapist providing home health services in Arbor Vitae, WI. You will help patients recover from injuries and manage chronic conditions while working across a rural territory that includes multiple small towns and lake communities. Expect to coordinate care in patients' homes and collaborate with interdisciplinary teams to support safe functional outcomes.

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
• Active Wisconsin physical therapy license required to start the assignment.
• At least 12 months of home health physical therapy experience within the past 3 years. (Client requirement.)
• BLS certification from AHA or ARC may be required.
• Experience with HCHB electronic medical record preferred.

• Previous travel or agency home health experience.
• Experience managing a rural caseload and traveling between patient homes.

  Monday through Friday.
• Territory includes Presque Isle, Manitowish Waters, Mercer, St. Germain, Eagle River, Land O Lakes, and Phelps.
• No on-call required.
• May require travel within the assigned territory between patient homes.

Your role will directly affect patients' ability to remain safe and independent in their homes. You will provide skilled evaluation, develop individualized treatment plans, monitor progress, and communicate with physicians and caregivers to optimize outcomes.

Arbor Vitae and the surrounding area offer access to lakes, forests, and outdoor recreation within short drives. The region is near the Eagle River Chain of Lakes and provides boating and fishing opportunities within a 30- to 60-minute drive.
