Licensed Practical Nurse

Cassia CONNECT seeks a Licensed Practical Nurse to provide compassionate, faith-based care to older adults in our senior living and skilled nursing communities. The LPN delivers hands-on nursing care, including medication administration, treatments, and monitoring of residents' conditions. You will collaborate with RNs, aides, therapy and other care partners to support personalized care plans, accurate documentation and regulatory compliance. Responsibilities

• Provide direct nursing care to older adults in skilled nursing and assisted living settings
• Administer medications and treatments according to provider orders and facility protocols
• Monitor, document, and report resident conditions, vital signs, and responses to treatment
• Support care planning, assessments, and interdisciplinary team communication
• Maintain accurate electronic health records and follow regulatory and infection-control standards
• Educate and support residents and families with compassion and respect for faith and values
• Collaborate with RNs, aides, therapy, and other team members to promote resident well-being
• Participate in shift reports, safety checks, and quality improvement initiatives
