The Scheduler is responsible for efficiently coordinating and maintaining the daily and weekly schedules of hospice clinical staff, ensuring patient needs are met in a timely and compassionate manner. This role requires strong communication skills, excellent attention to detail, and the ability to work in a fast‑paced environment while supporting both patients and clinical teams.
Duties and Responsibilities Scheduling & Coordination- Develop, manage, and adjust daily schedules for nurses, aides, social workers, chaplains, and other interdisciplinary team members.
- Ensure appropriate staffing coverage by matching patient needs with staff skill sets, territory assignments, and visit frequency requirements.
- Communicate schedule changes promptly to clinical staff, patients, caregivers, and leadership.
- Coordinate on‑call and weekend schedules as needed.
- Enter new admissions, visit requests, and clinical orders into the scheduling system accurately and promptly.
- Prioritize urgent or same‑day visit requests based on patient condition and regulatory requirements.
- Collaborate with the clinical team to ensure continuity of care and adherence to care plans.
- Maintain accurate and timely schedule records within the EMR.
- Serve as the primary point of contact for scheduling‑related inquiries from staff, patients, and families.
- Notify leadership of trends, gaps, or issues related to staffing or coverage.
- Monitor daily workflow to ensure compliance with hospice regulatory standards and internal policies.
- Support on‑call staff with updates and changes during evenings or weekends if part of the assigned rotation.
- High school diploma or equivalent required; associate’s degree preferred.
- Previous scheduling experience in healthcare, hospice, or home health strongly preferred.
- Experience with EMR systems and scheduling software is a plus.
