Position: Licensed Practical Nurse (LPN) – Hospice

Licensed Practical Nurse (LPN) – Hospice (Healthcare)

The Hospice LPN plays a vital role in supporting patients and families through end‑of‑life care, ensuring comfort, dignity, and high‑quality clinical support. This position collaborates closely with the interdisciplinary team to deliver coordinated, patient‑centered care in the home setting.

Essential Duties Patient Care & Coordination
  • Oversee and coordinate hospice homecare services for assigned patients and their families
  • Monitor and assess patient status, reporting changes promptly and making recommendations for adjustments to the plan of care
  • Implement care plans in collaboration with the interdisciplinary team (IDG)
  • Provide direct nursing care, symptom management, and comfort measures in accordance with hospice standards
  • Educate patients and families on hospice philosophy, disease progression, medications, and end‑of‑life expectations
  • Support patients and caregivers with compassion, empathy, and respect
Team Leadership & Collaboration
  • Serve as a resource and mentor for LPNs and other team members, offering guidance and support
  • Collaborate with RNs, social workers, chaplains, CNAs, volunteers, and other disciplines to ensure seamless care
  • Act as a liaison between the hospice team and external healthcare professionals, including physicians and facility staff
  • Participate in interdisciplinary team meetings to discuss patient needs and recommend improvements
Documentation & Compliance
  • Maintain accurate, timely, and complete documentation of patient care, assessments, and progress
  • Ensure compliance with all regulatory, organizational, and hospice‑specific policies and procedures
  • Uphold standards of infection control and universal precautions
Quality & Professional Development
  • Continuously evaluate hospice homecare services and contribute to quality improvement initiatives
  • Participate in ongoing professional development to stay current with best practices in hospice and palliative care
  • Demonstrate strong communication, time‑management, and problem‑solving skills
  • Represent the organization professionally in all interactions with patients, families, and healthcare partners
Education and/or Experience
  • Current, active, and unrestricted LPN license
  • Experience in hospice, palliative care, home health, geriatrics, or related field preferred
  • Strong clinical assessment and communication skills
  • Ability to work independently with minimal supervision while collaborating effectively within an interdisciplinary team
  • Compassionate, patient‑centered approach to care
  • Proficiency with electronic documentation and basic computer systems
  • Valid driver's license and reliable transportation for home visits
Physical Demands
  • Ability to safely lift, move, and transfer patients using proper body mechanics; may require lifting or supporting up to 50 pounds
  • Frequent standing, walking, bending, kneeling, and reaching during patient care
  • Ability to assist with mobility, repositioning, and comfort measures in varied home environments
  • Stamina to sustain prolonged physical activity and respond effectively during urgent situations
  • Ability to drive between patient homes or facilities as required
  • Adequate visual, auditory, and tactile acuity to assess patient conditions and perform clinical tasks safely
