Schedule and LocationsThis role will support our new clinics in Goodlettsville and Clarksville. The proposed schedule is for 3 days per week (Monday, Wednesday, Thursday), with two days at the Goodlettsville clinic and one day at the Clarksville clinic.

- Identify and assess patient’s clinical issues and health care needs.
- Record physical findings, formulate plan and prognosis based on patient’s condition and the doctor’s treatment plan.
- Conduct evaluations and re‑evaluations of established patients.
- Refer patients to physician for consultation or to specialized health resources when needed.
- Discuss the case with physician and other health professionals to prepare comprehensive patient care plans.
- Perform pain management injections for specific diagnoses according to practice protocols.
- Prescribe/Order diagnostics, therapeutics, medication, pain management procedures, and DME as medically indicated.
- Work independently as well as part of a team, with flexibility, initiative, and willingness to learn.
- Support the vision and culture of the organization; demonstrate commitment through participation in performance improvement processes.
- Perform all other duties as assigned.
Skills and Abilities
- Highly proficient with computers, especially EMR software, Meaningful Use, Microsoft Word, Excel, and Outlook.
- Ability to manage high levels of stress and make sound decisions.
- Very structured, organized, and professional personality; punctual and with excellent attendance.
- Active, valid (in state of clinic) APRN Nurse Practitioner certificate (required).
- DEA registration (preferred).
- Valid, unrestricted professional license in state of Indiana.
- Ability to obtain clinical privileges to practice medicine.
The physical demands of this position include sitting and standing with occasional light to medium lifting. Ability to help lift patients and transport in a wheelchair from the clinic to the parking lot is required. An employee must meet these requirements to successfully perform the essential duties of this job; reasonable accommodation will be made in accordance with ADA rules and regulations.
