We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team at Harbor Health & Rehab in East Chicago, Indiana. As an LPN (Day Shift), you will play a vital role in providing exceptional care to our residents while working closely with our interdisciplinary team to ensure their physical, emotional, and psychological well‑being.

• Provide direct patient care to residents in a skilled nursing setting, including administration of medications, wound care, and vital sign monitoring.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Conduct thorough assessments, document patient progress, and communicate effectively with residents, families, and colleagues.
• Collaborate with physicians, therapists, and other healthcare professionals to optimize patient outcomes.
• Maintain accurate and detailed records of patient care and treatment.
• Provide emotional support and comfort to residents and families.
• Participate in quality improvement initiatives to continuously enhance patient care and outcomes.
