Pride in what we do and a focus on patient care. As part of Connecticut’s largest healthcare network, the Team Coordinator role supports our Person‑Centered Care Model by scheduling patient visits, coordinating interdisciplinary teams, and ensuring quality and regulatory compliance.

Responsibilities
  • Perform duties in compliance with regulatory agencies, achieving deficiency‑free surveys.
  • Schedule nursing, therapy, and social work visits.
  • Facilitate timely initiation of care through accurate scheduling and problem solving.
  • Triage incoming calls using critical thinking, prioritization, and superior customer service skills.
  • Coordinate with supervisors and team members regarding patient information, data management, and regulatory compliance statuses.
  • Utilize and manage electronic and paper data, coordinating administrative functions to ensure accurate, efficient, and timely flow of information and financial data for interdisciplinary teams.
  • Provide administrative and clerical support to care management teams electronically and manually.
  • Demonstrate proficiency in computer navigation and medical/financial database communications.
  • Electronically assign and de‑assign clinical staff to patients by accurately utilizing system permissions.
  • Coordinate initial admission services and act as liaison for the clinical team.
  • Demonstrate effective communication, organizational, and time‑management skills.
  • Comply with agency processes to maintain standards and ensure standardization.
  • Organize daily work and information to maintain efficiency and confidentiality.
  • Ensure required information is provided and flows timely to complete billing, payroll, admissions, and care delivery functions.
Qualifications
  • Education:

    High School diploma or equivalent;
    Associate's Degree with a medical secretarial background preferred.
  • Experience:

    Three years of related office experience in a health‑related field preferred.
  • Skills:

    Knowledge of office procedures, computer proficiency, and effective communication.
