As part of the person-centered care model, this role will manage and deliver wound care for the Wound program. The role includes:
- Aid in the delivery of outcome-based wound care and cost effective care by utilizing metric-driven decisions.
- Collaborate with the Wound Team RN and CWOCN to achieve exceptional outcomes.
- Perform wound care in daily service delivery and maintain clinical care standards for clients receiving specialized wound care treatments.
- Promote daily operations and delivery of wound care to patients, and participate in the development, maintenance and implementation of program goals and objectives.
- Ensure consistent delivery of quality care and communication.
- Perform wound care to promote cost effectiveness for wound treatments in relationship to patient needs.
- Seek consultation with the Wound Team RN and WOCN staff for patient problems/issues requiring changes in the plan or interventions to solve complex problems involving client care.
- Participate in the interdisciplinary approach to client care with Wound program/Agency staff.
- Perform wound care and medical/surgical services required by the patient to achieve quality outcomes.
- Inform the Wound Team RN, CWOCN and the physician/wound clinics of changes in client needs and wounds requiring changes to the plan of care.
- Assist with staffing needs to meet service demands.
- Accountable for team performance in achieving desired clinical and operational performance measures.
- Follow trends in the specialty area of wounds for home care industry and make recommendations of treatment options for incorporation into the program.
- Participate in training, coaching of staff, onboarding, and the facilitation of professional development needs and competency for the wound program.
- Actively participate in the performance improvement process known as H3W.
- Participate in clinical education area to provide programs aimed at improving clinical competencies of direct service staff.
- Research new information related to wound specialty and communicate to the team.
- Perform other duties as assigned.
Education
CT approved Licensed Practical Nursing program required. Wound/Ostomy Assistant certification preferred.
Experience
Minimum of two years nursing experience in clinical specialty area required. Two years in homecare preferred.
