About the PositionFirst Choice Health Centers is seeking a Psychiatric Nurse Practitioner (PMHNP) for our community health practice in East Hartford, CT. This position is accountable for independently performing a full range of tasks in delivery of direct patient mental health care in an outpatient setting.

- Proven experience as a Nurse Practitioner implementing treatment plans, educating patients, and administering and managing patient medication(s).
- A current license to practice as a Nurse Practitioner in the state of Connecticut.
- A commitment to serving diverse and vulnerable populations with compassion and respect.
- Excellent communication, interpersonal, and organizational skills.
- Connecticut Controlled Substance license and DEA license.
- BLS Certification through the American Heart Association or American Red Cross.
- Pay is based on several factors including but not limited to work experience, certifications, etc. Range for this position is $130,000 - $185,000.
Job Duties
- Diagnose patient conditions and provide appropriate treatment recommendations.
- Interview patients about symptoms and complaints and determine whether a psychiatric disorder has a physiological or emotional basis. Takes medical histories.
- Assess risk factors for mental illness and broader wellness evaluations.
- Instruct and counsel patients regarding compliance with prescribed therapeutic regimens.
- Develop and implement patient management plans, record progress notes, and assist in provision of continuity of care.
- Assume responsibility for patient follow-up, coordination, and referrals to other health care providers as necessary.
- Maintain patient medical records.
- Perform related duties as required.
