This position primarily supports a skilled nursing facility with a medically complex patient population, including a ventilator unit. Experience caring for patients with pulmonary conditions or ventilator-dependent residents is strongly preferred but not required.

Compensation and Benefits- Earning potential: $110,000–$200,000 (salary based on experience + easily achieved monthly bonuses)

- Autonomy to deliver independent care with access to physician-level consults
- Deliver person-centered medical care using evidence-based practices
- Manage acute and chronic medical conditions in the skilled nursing setting
- Provide care for medically complex patients, including residents with chronic pulmonary conditions and ventilator needs, as appropriate
- Collaborate with nursing, respiratory therapy, and interdisciplinary facility teams
- Conduct assessments, develop treatment plans, and manage follow-up care
- Educate patients, families, and staff
- Complete timely documentation (within 24 hours)
- Ensure compliance with state regulations in skilled nursing facilities
- Active Nurse Practitioner (NP) or Physician Assistant (PA) license
- Minimum of two years of post-certification clinical experience
- Experience in skilled nursing facilities, hospitalist roles, internal medicine, emergency medicine, ICU, or pulmonary medicine preferred
- Experience caring for medically complex or ventilator-dependent patients is a plus
- Proficiency with EHRs and clinical technology
- Strong communication and collaboration skills
