Nurse Practitioner or Physician Assistant - House Calls

Seeking a Nurse Practitioner or Physician Assistant for in-home assessments within a flexible, non-prescribing role.

• Conducting comprehensive in-home health assessments, including medical history, vital signs, and screenings.
• Documenting diagnoses and collaborating with primary care providers to address care gaps.
• Identifying urgent situations and educating members on health management.
• Addressing social determinants of health and providing referrals.

• Active licensure and certification (ANCC, AANP, NCCPA) in relevant fields.
• Unrestricted driver’s license, reliable transportation, and ability to navigate home environments.
• Bilingual in Spanish preferred.

• Experience in clinical settings, proficiency with electronic records, and strong communication skills.
• Ability to transport equipment and navigate stairs during home visits.

• Range from $109,500 to $164,000, including base pay and bonuses.
