  PRN, Day shift

• Support patients, families, and loved ones during their end-of-life journey.
• Perform comprehensive assessments to establish goals for symptom management and end-of-life care.
• Implement effective case management strategies.
• Collaborate as a member of an active interdisciplinary team to address patients' needs holistically.
• Develop and maintain therapeutic relationships through effective communication with patients and families.
• Utilize state-of-the-art technology to document skilled services provided.

• Education: Registered Nurse with Valid CT RN license
• Experience: One year of RN experience;
  Homecare or Hospice experience preferred
• Licenses &
  
  Certifications:
  
  Valid drivers' license and reliable transportation required;
  Computer proficiency required

• Automatic enrollment into the Nurse Residency Program for all graduate RNs with less than one year of experience
