About This Role

The Hospice Nurse Weekend (RN) provides skilled nursing care to hospice clients in their homes or community health settings, ensuring high-quality, cost-effective care. This role is ideal for compassionate nurses who want to make a real difference in the lives of patients and their families.

• Location: East Hartford, CT
• Department/Unit: 95005-Hartford Hospice Gen
• Schedule: Part Time, Day shift

• Provide skilled nursing care to hospice clients in home or community settings.
• Review referrals and initiate contact with clients and families.
• Conduct complete nursing assessments based on referral data and clinical observations.
• Develop care plans in collaboration with clients and interdisciplinary teams.
• Facilitate peaceful death and ensure appropriate bereavement follow-up.
• Modify care plans as conditions change.
• Utilize interdisciplinary team members to support patient/family needs.
• Teach patients and reinforce caregiver education.
• Direct LPNs and Home Care Aides in executing the care plan.
• Participate in client care conferences and staff meetings.

• Associate’s degree in nursing required
• BSN preferred

• One year nursing experience required
• Home care experience preferred

• License to practice in the State of Connecticut required
