Licensed Practical Nurse, Psych Visits, PRN

• Verify the Plan of Care before each visit and provide care according to RN delegation, physician orders, assessment data, and established standards
• Incorporate patient care goals into nursing visits and document care on skilled nursing notes
• Communicate significant findings, problems, and patient or environmental changes to the RN Case Manager, physician, clinical supervisor, and involved personnel
• Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician
• Implement patient safety plans using patient, family, and community resources
• Teach patients appropriate self-care techniques while delivering quality care
• Suggest revisions to the Plan of Care and notify the RN Case Manager
• Provide specialized nursing services under the direction of a Registered Nurse or physician
• Collaborate with the RN Case Manager during business hours
• Document medication regimens and update medication profiles
• Treat patients and caregivers effectively and respectfully
• Take actions to prevent avoidable hospitalizations and ER visits
• Provide supportive communication to physicians, patients, families, visitors, and agency personnel
• Report to the Clinical Team Manager

• High School Diploma or GED
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• Current state license as a Licensed Vocational / Practical Nurse
• 1 year of experience in a clinical care setting
• Hospice or home health experience preferred
• 50% travel required
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Valid driver's license and insurance
• Reliable transportation to perform job tasks

Demonstrates expertise in patient care management, including the ability to implement care plans, communicate effectively with healthcare teams, and provide specialized nursing services under supervision. Proficient in documenting care and medication regimens while ensuring patient safety and quality care delivery.
