• Provide skilled nursing care to clients in home and community-based settings.
• Maintain client records utilizing the Agency's standard for documentation.
• Make follow-up visits to assess disease progress and evaluate response to treatment.
• Teach patients and reinforce client/caregiver education.
• Coordinate care with providers and consult with physicians regarding client needs.
• Direct LPNs and Home Care Aides in carrying out the established plan of care.
• Refer clients/families to community resources as needed.
• Attend and participate in in-service education programs.

Education:

• Graduate of an Associate's Degree or diploma program in nursing
• Graduate of NLN approved BSN program preferred

Experience:

• One year nursing experience or one year nursing experience in a specialty
