The Weekend OASIS Registered Nurse (RN) position at Hartford Health Care involves providing skilled nursing care to clients in their homes or community health settings.   Part Time, Day shift

• Provide skilled nursing care to clients in their homes or community health settings.
• Review referrals and initiate contact with client and family.
• Conduct complete nursing assessments based on referral data and clinical observations.
• Complete OASIS assessments with accurate scoring of clinical and functional questions.
• Develop a plan of care in collaboration with clients and all disciplines.
• Provide OASIS time point assessments throughout a patient’s episode.
• Make follow-up visits to assess disease progress and evaluate response to treatment.
• Teach patients and reinforce client/caregiver education.
• Direct LPNs and Home Care Aides in carrying out the established plan of care.
• Participate in health care programs and complete reporting requirements on a timely basis.

• Education:
  • Associate’s Degree or diploma program in nursing required
  • BSN preferred
• Experience:
  • One year nursing experience required
  • Home care experience preferred

  • Current RN License in Connecticut required
