About This Role

The Hospice Weekend RN provides skilled nursing care to hospice clients in their homes or community health settings, ensuring high-quality, cost-effective care. This role is ideal for nurses who are passionate about end-of-life care and want to make a meaningful impact on patients and their families.

• Location: East Hartford, CT
• Department/Unit: 95005-Manchester Hospice Gen
• Schedule: Part Time, Day shift

• Provide skilled nursing care to hospice clients in their homes or community health settings.
• Review referrals and initiate contact with clients and families.
• Conduct complete nursing assessments based on referral data and clinical observations.
• Develop a plan of care in collaboration with clients and all disciplines.
• Facilitate peaceful death and assure appropriate bereavement follow-up.
• Modify plan of care as conditions change.
• Utilize interdisciplinary team members to support patient/family needs.
• Teach patients and reinforce client/caregiver education.
• Direct LPNs and Home Care Aides in carrying out the established plan of care.
• Refer clients/families to other community resources as needed.

Education:

• Graduate of an Associate’s or diploma program in nursing
• Graduate of NLN approved BSN program preferred

Experience:

• One year nursing experience OR one year nursing experience in a specialty
• Medical/surgery or home
