The Weekend On Call Visiting Nurse (RN) provides essential nursing care in home settings, focusing on patient-centered care and integrated support. This role is ideal for nurses who thrive in dynamic environments and are committed to making a difference in patients' lives.

• Location: East Hartford, CT
• Schedule: Part Time, On Call (Friday 4:30 PM
  - Saturday 8 AM, Saturday 4:30 PM
  - Sunday 8 AM)

• Assume responsibility for evening, night, weekend, and holiday on-call duty as scheduled.
• Maintain regular communication with Nurse Supervisors, attending physician, and case manager.
• Respond to medical or emotional crises and provide direct care as appropriate.
• Conduct complete nursing assessments based on referral data and clinical observations.
• Develop and modify a plan of care in collaboration with clients and all disciplines.
• Initiate and maintain client records utilizing the Agency's documentation standards.
• Make follow-up visits to assess disease progress and evaluate response to treatment.
• Teach patients and reinforce client/caregiver education.
• Consult with physicians regarding client needs and plans of care.
• Direct LPNs and Home Care Aides in carrying out the established plan of care.

• Graduate of an Associate’s or diploma program in nursing required.
• Graduate of NLN approved BSN program preferred.

• One year nursing experience or one year nursing in a specialty required; medical/surgery or home care.

• Current RN License in Connecticut required.
