Community Health Resources, Inc. (CHR) seeks a TSEA Specialist to support emerging adults transitioning from foster care in East Hartford.
This full-time role features Monday–Friday, 8AM–8PM flexibility and collaboration with a dedicated care team. You will provide case management, crisis intervention, and trauma-informed care, while maintaining EMR documentation and collaborating with clinicians to help clients achieve stable housing, education or employment, and essential life skills.
