Hospice Registered Nurse Case Manager

Location:

Eaton Rapids County.

Employment Type:

Full-Time.

Job Responsibilities
  • Ensure all activities align with the vision of Elara Caring’s leadership and Hospice team.
  • Assess hospice patients and families to identify physical, psychosocial, and environmental needs using documentation, clinical records, Interdisciplinary Team reports, after‑hours reports, and on‑site evaluations.
  • Obtain data on physical, psychological, social, and spiritual factors influencing patient/family/caregiver health status.
  • Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved with patient care.
  • Counsel, instruct, and involve the patient, facility, and family in following the Interdisciplinary Plan of Care.
  • Teach hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospitals, assisted‑living facilities, etc.).
  • Provide appropriate support at time of death and perform bereavement assessment.
Qualifications
  • Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate school of nursing.
  • Current State License as a Registered Nurse (RN).
  • At least 1 year of experience in a clinical care setting.
  • Experience in a hospice or home health environment preferred.
  • 50% travel required.
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
  • Valid driver’s license and reliable transportation for job tasks.
Reporting

You will report to the Clinical Team Manager.
