Hospice Registered Nurse Case Manager
Location:
Eaton Rapids County.
Employment Type:

- Assess hospice patients and families to identify physical, psychosocial, and environmental needs using documentation, clinical records, Interdisciplinary Team reports, after‑hours reports, and on‑site evaluations.
- Obtain data on physical, psychological, social, and spiritual factors influencing patient/family/caregiver health status.
- Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other personnel involved with patient care.
- Counsel, instruct, and involve the patient, facility, and family in following the Interdisciplinary Plan of Care.
- Teach hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospitals, assisted‑living facilities, etc.).
- Provide appropriate support at time of death and perform bereavement assessment.
- Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate school of nursing.
- Current State License as a Registered Nurse (RN).
- At least 1 year of experience in a clinical care setting.
- Experience in a hospice or home health environment preferred.
- 50% travel required.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
- Valid driver’s license and reliable transportation for job tasks.
You will report to the Clinical Team Manager.
