About the Role:

As a Licensed Practical Nurse (LPN) at Aspen Glen Nursing and Rehabilitation Center, you will play a vital role in delivering high-quality, compassionate care to our patients. Your expertise and dedication will enable our team to provide exceptional support to patients, families, and caregivers. Licensed Practical Nurse (LPN) is responsible for:

  • Administering medications and treatments as prescribed by physicians
  • Conducting vital sign checks and monitoring patient condition
  • Assisting with patient admissions, transfers, and discharges
  • Maintaining accurate and up-to-date patient records
  • Collaborating with other healthcare professionals to develop and implement individualized care plans
  • Providing emotional support and education to patients and their families
Responsibilities:
  • Provide direct patient care, including administering medications, performing treatments, and monitoring patient condition
  • Collaborate with registered nurses and other healthcare professionals to develop and implement individualized care plans
  • Conduct daily rounds to ensure patient needs are met and report any concerns to the charge nurse
  • Maintain accurate and up-to-date patient records, including vital signs, medications, and treatment plans
  • Assist with patient admissions, transfers, and discharges, ensuring a smooth transition to/from the facility
  • Participate in quality improvement initiatives and maintain a clean and safe environment
  • Provide emotional support and education to patients and their families, promoting independence and autonomy
