Position: LICENSED PRACTICAL NURSE

Location: East Longmeadow Skilled Nursing Center, East Longmeadow, MA 01028, USA

Pay: $35 - $40 USD per hour.

Job Description

Licensed Practical Nurse (LPN) to provide nursing care to residents. Responsibilities include monitoring vital signs, inserting catheters, dressing patients, administering medications, handling emergencies, and coordinating with physicians and the Director of Nursing (DON). Document care, maintain accurate medication records, verify resident identity before medication, and give thorough shift reports.

Essential Job Functions
  • Provide basic nursing care such as checking blood pressure, inserting catheters, and helping comfort patients by performing tasks such as changing and dressing.
  • Document accurate and ongoing assessment of patient status.
  • Teach patient or significant other appropriate health information in a timely manner.
  • Perform direct patient care using established procedures, policies, and standards.
  • Prepare and administer medications according to policy and procedure, observe and document patient responses.
  • Demonstrate ability to handle emergency situations promptly and professionally.
  • Admit, transfer, and discharge residents as required.
  • Receive phone orders from physicians and record them on the Physician’s Order Form.
  • Chart all reports of accidents/incidents involving residents and follow established procedures.
  • Verify the identity of each resident before administering medication or treatment.
  • Ensure adequate medications, supplies, and equipment are available; report needs to the DON.
  • Maintain accurate narcotic records and notify DON of any discrepancies.
  • Give a thorough, concise report at shift change and communicate pertinent information in a timely manner.
Required License
  • Active Licensed Practical Nurse (LPN) license in the state of Massachusetts.
Qualifications
  • Technical or other training in Nursing or a related field.
  • License to practice as a Practical Nurse.
