Center for Human Development (CHD) is currently looking for Behavioral Health Licensed Practical Nurses (LPN) to join Heron House, an Enhanced Medical Group Living Environment (EMGLE) program. This 18-bed facility located in East Longmeadow, MA, allows you to provide quality care and support to those we serve with a small LPN to client ratio. Under the supervision of a designated charge nurse or RN, you will work with a support team of LPNs, CNAs, and Direct Care staff providing care and compassion while gaining valuable experience in CHD’s modern EMGLE facility.

LITTLE ABOUT THE EMGLE FACILITY

The Enhanced Medical GLE (EMGLE) provides a variety of skilled healthcare and supportive services including around‑the‑clock nursing and personal care assistance to persons with mental illness in addition to their medical needs. These services are designed to meet the daily needs of persons with chronic medical conditions, terminal illnesses, and/or disabilities impacted by significant mental illness. The model also serves to maintain, prevent, or slow the worsening of condition and assist transition to a lower level of care or prevent hospitalization.

A competitive hourly wage of $38.50/hr with a $2.00/hr differential for overnight shifts.

Additional holiday bonus pay is provided for working on New Year's Eve, Fourth of July, Labor Day, Memorial Day, and Christmas Day. Take advantage of an attractive benefit package that is available with this LPN opportunity.

Schedule

Now hiring one shift, 7am‑3pm, part time (16 hours a week). Schedule is a two‑week rotation.

Week 1:
Wednesday and Saturday: 7am‑3pm;
Week 2:
Sunday and Friday: 7am‑3pm.

We also have per‑dem hours available for 1st, 2nd and 3rd shifts.

Your role as an LPN
  • Medication administration and management; assist in medication audit processes.
  • Attend participant care plan meetings/resource meetings.
  • Provide direct care as needed, including personal hygiene and bathing assistance.
  • Effectively understand patient health details and provide clear, precise information and observations to charge nurse or RN.
  • Assist with activities of daily living: serving meals, feeding, ambulating.
  • Administer nonsterile dressings, surgical preps, ice packs, heat treatments.
  • Track vital signs, weight, intake and output information to maintain stability.
  • On‑call monthly rotation (stipend provided, plus hourly wage if called for medication administration).
Qualifications
  • Licensed practical nurse by the Board of Registration in Massachusetts.
  • One‑year experience as an LPN (preferred).
  • One‑year experience in a behavioral health setting (preferred).
  • One‑year experience working with the elderly, people with disabilities, behavioral challenges, and cognitive disabilities (preferred).
  • Valid driver’s license.
  • Ability to lift 45 lbs. or more and meet the physical demands of the role.
  • Complete assigned training requirements in compliance with CHD and the Massachusetts Department of Mental Health.
