Responsibilities- The MDS Coordinator is responsible for the accurate and timely completion of all Medicare/Medicaid case-mix documents to assure appropriate reimbursement for care and services provided within the Facility.
- Conducts continual Minimum Data Set (MDS) reviews to assure achievement of optimal allowable Resource Utilization Group (RUG) category.
- Oversees the overall process and tracking of MDS/PPS documentation and submission.
- Integrates nursing, dietary, social recreation, restorative, rehabilitation and physician services to ensure appropriate assessment and reimbursement.
- Develops, implements, monitors and supervises the restorative nursing program; acts as liaison between formalized therapy and nursing.
- Promotes a restorative nursing philosophy among facility departments.
- Assesses and determines the health status and level of care of all new admissions.
- Ensures the accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, and significant change.
- Communicates level of care for new residents to all disciplines.
- Coordinates interdisciplinary participation in completing the MDS for each new admission according to regulatory time frames; ensures documentation meets federal, state and medical standards.
- Maintains an accurate schedule of all MDS assessments with proper reference dates throughout the resident’s stay.
- Responsible for data entry and electronic submission of MDS assessments; verifies electronic submissions and performs corrections as needed.
- Schedules and conducts resident care conferences in compliance with regulations and ensures completion of all MDS reviews prior to conferences.
- Assists disciplines in formulating and revising care plans; identifies and prioritizes problems with appropriate nursing interventions.
- Evaluates resident care plans for comprehensiveness and individuality; reassesses and revises goals as needed.
- Coordinates level of care changes within the facility and notifies all departments as changes occur.
- Generates forms for level of acuity changes and transmits them to the appropriate agency as required by state law.
- Participates in development, organization and implementation of nursing care philosophy, objectives, policies, procedures and standards in line with facility goals.
- Confers with the Director of Nursing/Assistant Director of Nursing regarding the quality of nursing care and opportunities for improvement.
- Makes daily rounds to evaluate patient care, monitor nursing practices, and assist Human Resources with nursing and educational needs.
- Maintains awareness of legal aspects of nursing practice and government regulations; demonstrates this in teaching, supervision, and evaluation of patient care.
- Performs rehabilitation assessments for new admissions and ongoing assessments on a quarterly basis as needed.
- Oversees restoration-related evaluations and plans of care; collaborates with therapy and nursing staff to ensure recommendations are carried through.
- Works closely with the therapy department to ensure recommendations are implemented after discharge from therapy.
- Reviews weekly documentation for residents receiving restorative services and ensures proper ADL documentation.
- Oversees incontinent management programs and attends care planning conferences to provide restorative nursing input.
- Coordinates with Human Resources and Director of Nursing to orient new staff to restorative nursing roles and practices.
- Coordinates with Director of Nursing and Therapy to provide ongoing education/evaluation of direct care staff in restorative nursing.
- Participates in physical restraint reduction and fall management programs.
- Sets up documentation flow sheets for daily restorative nursing documentation and oversees restorative nursing staff.
- Serves as a liaison with all therapists (physical, occupational, speech) and participates in care planning and CMI meetings.
- May serve in the role of Medicare Services Coordinator (MDS Coordinator) as needed.
- Puts patients first and ensures high-quality service in a caring and compassionate atmosphere that respects individual needs and rights.
- Other duties as assigned.
- Registered Nurse with current unencumbered state license.
- 3 - 5 years of equivalent clinical experience.
- Current CPR certification is required.
- Long Term Care experience preferred.
- Ability to read, write, speak and understand the English language.
- Required to sit, stand, bend and walk regularly; lift and/or move up to 25 pounds.
- Visual and auditory ability sufficient for written and verbal communication.
- The noise level in the work environment is usually moderate.
