The Social Services Director plays a critical role in planning, organizing, implementing, evaluating, and directing the overall operation of the facility's Social Services Department. This role ensures the medically related emotional and social needs of residents are met individually while maintaining compliance with federal, state, and local standards, guidelines, and regulations.
Essential Duties & Responsibilities- Review facility policies and procedures as part of the interdisciplinary team to ensure compliance with federal and state regulations
- Participate in developing and setting policies for resident care, quality of life, and social work operations
- Attend and contribute to quality assurance and interdisciplinary team meetings
- Maintain compliance with all government requirements for social service documentation
- Document progress in meeting residents' psychosocial needs
- Work with residents, families, significant others, and staff to promote resident rights, self-advocacy, and quality of life
- Complete social histories and psychosocial assessments for each resident
- Develop individualized, interdisciplinary care plans addressing psychosocial needs and appropriate interventions
- Provide or coordinate therapeutic interventions to support residents' adjustment to long-term care
- Educate residents, families, and staff on residents' rights, cultural diversity, and available community, health, and social services
- Provide or coordinate resident/family support groups as appropriate
- Respond to catastrophic events and provide clinical interventions as needed
- Coordinate discharge planning and make referrals to appropriate home care or social services
- Supervise social services students and document their progress per school guidelines
- Bachelor's degree in social work or related field preferred
- Minimum 2 years of social services experience
- Long-term care experience preferred
- Strong communication and interpersonal skills
- Ability to sit, stand, bend, and walk regularly
- Ability to lift/move up to 25 pounds
- Visual and auditory ability sufficient for written and verbal communication
- Moderate noise level in the work environment
