Manager, Continuity of Care & Performance Improvement – Hospice Position Summary
VNA Health Group is seeking an experienced RN leader to oversee after-hours hospice operations, ensuring continuity of care across evenings, nights, weekends, and holidays. This role is responsible for clinical quality, regulatory compliance, staff oversight, performance improvement, and timely response to patient and family needs.

- Monitor quality, compliance, response times, and performance metrics
- Conduct clinical documentation audits and identify improvement opportunities
- Lead performance improvement initiatives and support survey readiness
- Serve as the escalation point for complex patient situations
- Assist with staff onboarding, education, and coaching
- Active NJ RN License required
- 5+ years of hospice experience
- 3+ years of leadership experience
- Hospice quality, audit, and compliance experience preferred
- Strong communication and organizational skills
- Valid driver's license required

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $92,000 to USD $120,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions.
