Full Time, Regular, Days, 8am to 4pm

Responsibilities
  • Conduct comprehensive and continuous assessment of deep wounds, skin tears, ostomies, severe burns, pressure injuries or bedsores, continence, and diabetic foot wounds to determine potential complications, immediate treatment needs, and long‑term treatment plans; collaborate with the health‑care team as needed.
  • Collaborate with clinical providers and the team to establish individualized, comprehensive plans of care for patients with complex wounds, ostomies, and/or continence conditions based on patient risks, diagnosis, and needs; create a care plan for the interdisciplinary team, including staff nurses.
  • Complete thorough wound‑care evaluations, document assessments, and provide treatment recommendations.
  • Evaluate patients’ responses to care in conjunction with physicians and modify pressure‑ulcer/wound/ostomy plans as needed.
  • Provide staff, patient, and family education and counseling regarding wound care, risk factors associated with pressure injury, infection prevention, treatment measures, and related issues; guide allied nursing personnel; refer acute / chronic skin alterations to appropriate specialty services (Surgery, Vascular, Podiatry, Dermatology, Plastic).
  • Develop and implement wound and skin‑care prevention and treatment programs; provide education to patients, family, and allied personnel; evaluate and monitor program effectiveness.
  • Participate in product evaluation for ostomy and skin‑care needs to ensure cost‑effective, quality care.
  • Participate in the development, evaluation, and revision of policies, procedures, and guidelines related to wound care based on patient needs, evidence‑based trends, and industry standards.
  • Collaborate with staff to establish protocols that support best practices; serve as a clinical expert and assist in educating staff nurses on wound assessment.
  • Observe, record, and interpret patient responses and wound status, reporting any changes; maintain nursing plans and progress notes.
  • Participate in departmental and interdisciplinary committees, conferences, nursing quality committees, and quality‑improvement initiatives related to wound, ostomy, and continence issues.
  • Develop policies, procedures, and programs facilitating delivery of evidence‑based practice to pressure‑ulcer/wound/ostomy patients.
  • Manage collection and analysis of all data related to hospital‑acquired pressure injuries; use quality measures to mitigate risk and drive improvement.
  • Provide in‑services on wound prevention, wound care, ostomy care, and incontinence.
  • Coordinate, supervise, and perform patient care as necessary.
Technical Skills
  • Ability to perform wound assessment, reassessment, and management for all patients.
  • Knowledge of anatomy, physiology, and disorders of the gastrointestinal, genitourinary, and dermal systems.
  • Follows the standards of the Organization of Wound Care Nurses.
  • Thorough knowledge of wound staging systems.
  • Skill to develop and implement policies and procedures to manage patients with draining wounds, fistulae, tubes, wound prevention, wound care, ostomy care, and incontinence.
  • Ability to revise plan of care based on patient response and evaluate overall effectiveness.
  • Provide treatments and services within nursing scope; formulate teaching plans and evaluate effectiveness.
  • Knowledgeable of medications and their correct administration.
  • Knowledgeable of clinical products currently available.
  • Operate all equipment correctly and safely.
Minimum Education /Certifications
  • BSN required.
  • MSN, APRN preferred.
  • Certified Wound Ostomy Continence Nurse (CWOCN) or Certified Wound Care Consultant (CWCC) required; additional certifications as needed.
  • Current or compact RN licensure in the state of New Jersey.
  • BLS, ACLS.
Minimum Work Experience

3 years of nursing experience required in hospital or LTACH.

Physical Requirements

Physically able to sit, stand, stoop, and walk for long periods. Frequently assist lifting patients and equipment. Able to work in a stressful environment, independently, with critical thinking and decision‑making. Able to push/pull and lift up to 50 lbs.

Working Hours

Monday to Friday 9:00 am to 5:00 pm; willing to work differential hours, holidays, weekends, and oversee the overnight shift as needed.
