Brookhaven Health Care Facility is seeking a dedicated Registered Nurse (RN) - Admission Coordinator to manage the admissions process for new residents and provide exceptional nursing care. This role is perfect for an RN with excellent communication skills and a passion for delivering high-quality care.

Key Responsibilities:

* Coordinate the admission process for new residents, including assessments and documentation.

* Conduct resident assessments and collaborate with interdisciplinary teams to develop care plans.

* Ensure all admissions meet facility policies, state, and federal regulations.

* Provide education to families and residents regarding services and procedures.

* Act as a liaison between families, medical staff, and the administrative team to ensure smooth transitions.

* Maintain accurate and up-to-date resident records in compliance with regulatory standards.

Qualifications:

* Current and active Registered Nurse (RN) license in New York State.

* Previous experience in admissions, case management, or a similar nursing role preferred.

* Strong organizational, communication, and interpersonal skills.

* Ability to work independently, manage multiple priorities, and ensure excellent patient care.

* Compassionate, team-oriented, and able to build strong relationships with residents and families.
