Overview
Amerita is a leading home infusion provider and a subsidiary of Phar Merica. We are growing in home infusion therapy and seek a Pharmacist to join our Pharmacy team. The Pharmacist will report to the Pharmacy Manager and work in our City, State branch. As a core member of the Pharmacy team, you will perform pharmaceutical care activities for customers and patients within the service area, with ongoing professional development and career progression discussions.
Schedule: Monday - Friday, 8:30am - 5:00pm

Responsibilities
- Evaluates and monitor drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals.
- Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.
- Adheres to all appropriate standards of accreditation and participates in quality assurance activities.
- Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service and distribution.
- Reviews, receives and verifies all new patient orders and documents on the respective order form.
- Assists the physician and other involved healthcare professionals with:
- Planning and implementing patient care in the home;
- Setting therapeutic objectives for drug therapy;
- Establishing parameters for monitoring the safety and efficacy of drug therapy; and
- Preparing regular progress reports.
- Conducts and documents initial and ongoing assessment of patient, drug and disease information to identify real and potential drug-related problems or needs.
- Monitors the outcomes of drug therapy by collecting, evaluating and interpreting laboratory data, clinical data and other pertinent patient information.
- Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
- Maintains thorough documentation in the patient record of all clinical activities and communication with patients, physicians and other healthcare professionals.
- Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.
- Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars and conferences.
- Compounds sterile and non-sterile preparations, including parenteral nutrition, antibiotics, chemotherapy, hydration and narcotic analgesics, using aseptic technique where state laws permit.
- Ensures compounding/clean room areas and related equipment are cleaned, sanitized and maintained in compliance with company policies and USP 797 and USP 800 standards.
- Documents all activities on appropriate logs.
- Pharmacists’ duties extend to overseeing the Pharmacy technician functions as required by Pharmacy Board rules.
Qualifications
- Bachelor of Science in Pharmacy required;
Doctor of Pharmacy preferred - Registered Pharmacist License in state(s) of practice
- Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy
- Experience with compounding infused drugs
- Proven record of evaluating and monitoring drug therapies for home care patients
- Good organizational and time-management skills
- Participation and membership in professional societies and organizations is desirable
- Excellent verbal and written communication skills
- Flexible schedule with the ability to work on-call evenings and weekends on an as-scheduled basis
- Travel: 0-25% – Minimal travel may be required
Physical Requirements
To perform this role, constant sitting and typing on a keyboard with fingers, with occasional standing and walking. Ability to push/pull and lift/carry 10-30 lbs.
