Hope Health Hospice & Palliative Care is seeking a dedicated Registered Nurse (RN) Case Manager to provide expert clinical oversight for hospice patients in home and facility settings, primarily in the Milton/Dedham/Westwood or Norwood area and surrounding communities.
The ideal candidate will hold an RN license (BSN preferred), have 1–3 years of clinical experience, strong assessment and communication skills, and a collaborative approach with physicians and care teams.
