Coordinate and track regularly scheduled Amerita nursing visits for field staff and subcontracted agencies;
Secure home health agency availability for patient visits when Amerita nurse cannot provide service;
Communicate patient demo and infusion orders to field nurses and update infusion orders as needed;
Confirm authorizations and reauthorizations for nursing visits are secured as needed;
Provide infusion treatment plans to treating providers and maintain communication log
