Registered Nurse (RN) - Home Care

At PACE-RI, we provide comprehensive care that enables older adults to live independently in their communities with dignity. Our Enrollment RN is at the very heart of making this happen. Working daily as a key member of our dedicated enrollment team and reporting to the Chief of Growth, you will serve as a welcoming clinical face for prospective families—building relationships with potential enrollees, conducting initial health assessments, and helping Rhode Island seniors navigate their transition into our care.

No two days in this role look the same. While based out of our East Providence center, you will travel to evaluate home safety, functional abilities, and clinical needs for prospective participants, focusing primarily on the Southern Rhode Island and Aquidneck area (mileage paid). You will work hand-in-hand with the enrollment team to review medical records for eligibility, assist with initial care planning, host community health clinics, and help our outreach staff understand clinical eligibility criteria.

From attending health fairs to partnering with hospital discharge planners, you will act as a vital clinical resource, advocate, and representative for our enrollment efforts.

This role requires an active Rhode Island RN license, an Associate’s degree in nursing (Bachelor’s preferred), a valid driver’s license, and reliable transportation. Experience in geriatric, home health, or adult day care settings is highly preferred, along with sharp clinical judgment, strong communication skills, and the ability to work autonomously in the field.
