RN Case Manager

Revive Therapeutic Services is seeking a dedicated and organized Registered Nurse (RN) Case Manager to join our team. This role focuses on care coordination, case management, and member support to ensure individuals receive appropriate services and achieve positive health outcomes. This position does not involve direct clinical care.

• Manage a caseload of approximately 80 members.
• Conduct assessments and develop individualized care plans.
• Coordinate services with healthcare providers, community resources, and support systems.
• Monitor member progress and update care plans as needed.
• Provide education, support, and advocacy to members and their families.
• Maintain accurate and timely documentation.
• Collaborate with interdisciplinary team members to ensure continuity of care.
• Participate in team meetings, supervision, and ongoing training.

• Active Registered Nurse (RN) license required.
• Case management, care coordination, population health, or related experience preferred.
• Strong organizational, communication, and time-management skills.
• Ability to work independently and collaboratively within a multidisciplinary team.
• Proficiency with electronic documentation systems.
• Full-time position.
• Hybrid schedule with two days per week in the office and the remainder working remotely.
