Registered Nurse; RN Home Care Admissions Nurse - Weekend

Job Summary

This position is a Home Care Admissions Nurse – Weekend Days (RN) with a $10,000 sign‑on bonus at Lehigh Valley Health Network.

Performs comprehensive subjective and objective initial and follow‑up assessments of patients’ status—including physical, psychosocial, and environmental parameters—to formulate individualized plans of care per physician orders. Administers skilled treatments, interventions, and patient education focused on end‑of‑life issues, palliative care, advance directives, chronic disease management, pain management, symptom control, home care, hospice, and discharge planning.

• Determine home health as the appropriate care level and skilled needs per Home Health Conditions of Participation.
• Conduct initial and ongoing assessments of patient status and develop individualized care plans.
• Deliver patient care based on the medical plan of treatment established by the physician and protocols, using a family‑centered approach.
• Provide educational opportunities for patients, families, and clinical staff on palliative care topics.
• Promote patient/caregiver autonomy and evaluate the effectiveness of teaching, modifying education as needed.

• Bachelor’s or Associate’s Degree in Nursing.
• 2 years of experience in a home care setting.
• 1 year recent direct care experience and 1 year Home Health experience with OASIS data set collection.
• Knowledge of patient education techniques and principles.
• Ability to work independently and as part of a care team.
• Valid driver’s license and willingness to travel to patient assignments.
• Proficiency in completing OASIS data set accurately.
• American Heart Association Basic Life Support certification (State of Pennsylvania).

• AMB‑BC – Ambulatory Care Nursing certification.
• ANCC certification (State of Pennsylvania) within 3 years.

Ability to lift and carry 40–50 lbs.; push/pull patients on bed or stretcher, perform lateral transfers, and walk 8–10 miles per shift. May require kneeling, bending, squatting, and CPR.

Weekend Shift (Fri–Sun) 12‑hour shifts, 8AM – 8:30PM. Primary location: 175 E Brown St, East Stroudsburg. Position type:
