LV HOME CARE AND HOSPICE REGISTERED NURSE (RN) - ON CALL HOSPICE NURSE - FULL-TIME EVENINGS
** Summary
* * Provides clinical support and assists in the coordination and delivery of patient care services during or after normal business hours. Performs data/order entry, clinical record reviews, and other clinical tasks related to the provision of patient care. Plans, organizes, and directs care in collaboration with the interdisciplinary team to meet the needs of the patient and family in their homes and communities.

** Job Duties
*** Performs telephone triage for patients calling during or after hours utilizing critical thinking skills and sound clinical judgement
* Performs assessments of patient status, provides direct nursing care, and addresses immediate needs related to physical, psychosocial, and environmental factors. Provides educational opportunities for patients, families, and clinical staff focusing on end-of-life issues, palliative care, advance directives, pain management, symptom control, home care, hospice, and discharge planning.
* Performs data/order entry, clinical record reviews and other clinical tasks related to the provision of patient care.
* Provides hand-off communication to RN covering next shift, interdisciplinary team members, and/or Clinical Manager as appropriate regarding triage information, patient status updates, and patients who require a follow up visit or phone call.
** Minimum Qualifications
*** Specialized Diploma NLN approved/accredited nursing program
* 1 year recent experience providing direct care in a healthcare setting
* Demonstrates knowledge and ability to care for patients based on the population served.
* Demonstrates good critical thinking skills and sound clinical judgement.
* Demonstrates analytical and problem-solving skills.
* Ability to perform duties with accuracy and attention to detail.
