About the Role
Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations and bridge gaps in care.
Responsibilities- Conduct in-home health evaluations lasting about 45 minutes, including medication & medical history review, physical evaluation, and, when ordered, additional diagnostic tests.
- Use an iPad and the intuitive clinical workflow.
- Bring compassion and purpose to each visit, build trusted relationships and improve health outcomes.
- Visit millions of people nationwide to connect them to appropriate care.
- Travel locally and within licensed states as needed; conduct virtual visits when required.
- Provide Diagnostic & Preventive Services products (spirometry, DEE, bone density screening, etc.).
- Active, unrestricted license(s) in coverage area(s).
- Board certification (required). Multi-state licenses preferred.
- Qualified to cover primary locations:
Wenatchee and surrounding area. - Part-time schedule; anticipated weekly hours 40.
Pay range $46.03 - $99.14 per hour. Includes base hourly rate or base annual full-time salary dependent on experience, education, geography and other factors.
