Transitional Support Program, East Windsor, CT
Transitional Support Program (TSP) is a Supported Apartment Program for Young Adults ages 18-25 staffed 24/7 by a multidisciplinary team including Employment Specialists, Therapists, Residential Counselors, Administrative Assistants, Residential Supervisors, and Program Directors. TSP provides both group and individual modalities focused on daily living, social, recreational, time management, interpersonal, conflict resolution, coping, and symptom management skills to help young adults transition to independent living.

- Maintain the physical environment and program standards in compliance with CHR, DMHAS, JCAHO, and other licensing requirements.
- Monitor medications of all residents in accordance with the adult residential medication policy and procedure.
- Use appropriate communication methods/tools to transfer information to other staff regarding daily events.
- Teach and assist residents in nutrition, meal preparation, hygiene, laundry, banking, budgeting, and basic home‑care skills.
- Assist and manage administrative functions and tasks at the direction of the supervisor.
- Prepare and facilitate educational and skill‑building groups and individual interventions as directed by the treatment plan.
- Collaborate and coordinate care with providers and families as needed.
- Assist residents in addressing medical and psychiatric needs.
- Maintain documentation in compliance with agency and program standards.
- Participate in staff meetings, supervision, trainings, and other meetings as assigned.
- Perform additional duties as required.
- High school diploma or GED required.
- Two years of experience working with a relevant population preferred.
- Valid driver’s license with a clean driving record required.

Compensation:
Starting at $19.00/hr minimum, commensurate with experience and internal equity policy.
