PSR-I A Case Manager

The Community Service Board of Middle Georgia's Emerging Adult program is seeking a qualified individual to serve as a PSR-I A Case Manager! Our PSR-I A Case Manager adheres to treatment plans and provide services and interventions as planned. A PSR-I A Case Manager will also be responsible for recognizing consumer needs and assisting them in obtaining appropriate services. PSR-I A Case Managers shows consideration for others, works cooperatively with any co-worker, provides constructive feedback without undue criticism of others;

displays appreciation of differences in approaches, personalities, and viewpoints of others. The Case Manager will work with all involved to ensure this team moves in the same direction; everyone stays informed and monitors needs and progress.

Responsibilities of the PSR-I A Case Manager:

  • Per standards for PSR-I/CSI, make required contacts per month to consumers.
  • Minimum of 2 per month however working with this population of consumer's daily contact may be needed
  • Adhere to treatment plans and provide services and interventions as planned. Write progress notes (24-48 hrs) and bill for services provided.
  • Adhere to DBHDD guidelines in regard to the quality of documentation.
  • Complete notes within agency policy timeline.
  • Recognize consumer needs and assist them in obtaining appropriate services.
  • Provide consumer information for outside resources and encourage them to access them as independently as possible.
  • Communicates accurate information to others in a professional and courteous manner; conveys a willingness to assist.
  • Shows consideration for others, works cooperatively with any co-worker, provides constructive feedback without undue criticism of others; displays appreciation of differences in approaches, personalities, and viewpoints of others.
  • Solicits input of those who are affected by plans or actions; gives credit and recognition to others who have contributed; demonstrates concern for treating people fairly and equitably.
  • Accepts responsibility for own mistakes and takes action to prevent similar occurrences; works to resolve conflicts and to identify solutions in which all parties benefit.
  • Identifies team goals and ways to work with coworkers to accomplish those goals; works to keep group activities productive/focused on results.
  • Demonstrates eagerness to learn and assume responsibility; seeks out and accepts increased responsibility; displays a "can do" approach to work.
  • Shows persistence and seeks alternatives when obstacles arise; seeks alternative solutions; does things before being asked or forced to by events.
  • Works within the system in a resourceful manner to accomplish reasonable work goals; shows flexibility in response to process changes and adapts to and accommodates new methods and procedures.
  • Accepts direction and feedback from supervisors and follows through appropriately.
  • Attend in-service training as required.
  • Attend monthly staff meetings.
  • Attend meetings with other agencies/court as requested.
  • Contact Supervisor/Clinical Supervisor to obtain guidance and assistance when necessary.
  • Review available professional literature and educational material in a timely manner to improve the level of knowledge/skills.
  • Complete all duties assigned in a timely manner.

  • High School Diploma/GED Equivalent
  • Valid Georgia Driver's License
  • Effective verbal and written communication skills
  • Strong interpersonal skills and the ability to work effectively with diverse communities
  • Ability to work independently and in collaboration with others
  • Experience with Microsoft 365 Office Products
  • Ability to organize, prioritize and meet deadlines accordingly

Here's what would put you over the top:

  • Bachelors Degree in a related field (Social Work, Psychology, Human Services, Etc.).
  • Previous experience with case management or behavioral health.

Final pay rate will be dependent on a combination of qualifications such as experience and education.
