Position Summary
Under the leadership of the Social Work Manager, the Masters Social Worker (MSW) works independently to assess the needs of participants and families/caregivers. The MSW is a member of the Participant Care Team (PCT) and the Interdisciplinary Team (IDT) and participates in the assessment, intervention, management, and review of social service needs. The MSW also assists the IDT in determining eligibility and care needs.
The MSW provides care through a variety of modalities, including bio‑psychosocial assessment, interventions that include individual and group counseling utilizing motivational interviewing, cognitive behavioral techniques, psychoeducation, consultation, and care planning/case management.
- Makes comprehensive bio‑psychosocial assessments upon enrollment and at 6‑ and 12‑month intervals thereafter.
- Completes acute assessments on an as‑needed basis to answer specific referral questions.
- Utilizes measurement‑based care.
- Develops and implements appropriate, individual treatment plans for participants and caregivers, including individual and/or group counseling, education, and consultation services to the care team.
- Conducts risk assessment and crisis intervention and directs case management issues such as housing, financial, and safety concerns, including potential abuse or neglect.
- Assess psychiatric concerns such as mood changes, suicidal or homicidal ideation, and substance use or misuse.
- Evaluates caregiver burden and determines the need for transfer to a higher level of care (e.g., behavioral health services).
- Co‑ordinates discharge planning activities and processes and 24‑hour care as indicated by the interdisciplinary team or the care team.
- Educates and counsels participants and/or caregivers regarding care needs, options, and related problems.
- Advocates on behalf of participants and resolves concerns or issues in a timely manner.
- Collaborates to develop, communicate, and update the plan of care with the IDT, participant, and/or caregiver.
- Completes timely and accurate documentation in the electronic health record (EHR).
- Communicates changes in participants with the IDT and family members and evaluates participant outcomes or progress toward care‑plan objectives.
- Performs other related duties as assigned by leader(s).
- Master’s degree in Social Work from an accredited school.
- Legally licensed to practice as a social worker in the State of Michigan (limited license may be considered based on experience and supervised hours).
- At least one year of clinical social work experience, preferably in geriatrics.
- Knowledge of human behavior principles and individual and group therapy techniques, preferably CBT and DBT, short‑term interventions, and substance‑use disorder assessment and treatment.
- Demonstrated ability to evaluate, plan, implement, and monitor patient‑centered approaches to care.
- Understanding of applicable local, state, and federal laws and regulations.
- Commitment to cultural competence and ability to care for a diverse population.
- Proficiency with electronic health record systems, Microsoft Word, Excel, Outlook, and PowerPoint.
- Positive, flexible, solution‑focused attitude and ability to collaborate effectively in a team environment.
