Key Responsibilities
- Assessment & Care Planning: Complete and submit accurate Minimum Data Set (MDS) assessments (scheduled and unscheduled) according to federal and state regulations.
- Reimbursement Management: Set Resource Utilization Groups (RUGs) levels to ensure maximum, accurate reimbursement.
- Interdisciplinary Team Coordination: Lead care planning meetings, collaborate with nursing, PT/OT, and dietary staff to review patient progress and update plans.
- Quality Assurance: Monitor patient care for quality indicators, ensuring compliance with state and federal regulations.
- Compliance: Conduct interviews and review medical records to ensure all documentation justifies the care provided and reimbursement level.
- Conducting assessments: Assess residents for functional, nutritional, behavioral, and physical needs.
- Updating records: Maintain records for all residents in the facility.
- Improving care: Work with other staff members to improve patient care.
- Investigating issues: Investigate complaints from family members or residents.
- Coordinating care: Lead meetings to develop care plans.
- Documenting: Record data on charts.
- Evaluating: Review residents' conditions.
- Assessing patients: Review charts.
- Communicating: Discuss care plans with family.
- Maintaining records: Submit documentation.
- Completing assessments: Ensure all assessments are done on time.
- Developing plans: Create care plans.
- Handling compliance: Submit paperwork for reimbursement.
- Performing clinical assessments: Conduct clinical assessments.
- Working with team members: Coordinate with team members.
- Maintaining a safe environment: Ensure environmental safety.
- Managing the assessment process: Coordinate the assessment workflow.
- Reviewing records for accuracy: Ensure documentation correctness.
- Following regulations: Adhere to all applicable regulations.
- Reviewing patient care: Monitor patient care services.
- License: Active Registered Nurse (RN) license; some facilities accept LPN with extensive experience.
- State Nursing License: Valid state nursing license; bachelor’s degree in nursing may be required.
- Experience: Previous experience in long‑term care (LTC) or skilled nursing facility (SNF) with in‑depth knowledge of MDS 3.0 and RAI processes.
- Knowledge: Deep understanding of Medicare/Medicaid regulations, ICD‑10 coding, care planning, Medicare reimbursement, and state and federal guidelines.
- Software: Experience with MDS software and electronic health record (EHR) systems.
- Assessment
Skills:
Strong assessment abilities, attention to detail, critical thinking, and proficiency in EHR. - Organization: Ability to manage timelines for assessments and documentation.
- Communication: Excellent communication skills with healthcare staff and families.
