• Provide professional nursing care to patients in their homes.
• Assess, develop, implement, and evaluate home nursing care needs.
• Demonstrate competency with infusion therapy.
• Ensure compliance with physician orders and documentation.
• Utilize an interdisciplinary team and document all activities accurately.
• Complete nursing assessments and medication reconciliations.
• Update care plans within two weeks of changes in patient status.
• Act as a liaison among physicians and healthcare team members.
• Respond to after-hours messages and perform unscheduled patient visits as necessary.
• Maintain awareness of new standards and incorporate them into nursing practice.

• Associate's Degree in Nursing or Diploma Certificate in Nursing

• Minimum of one (1) year of RN experience

• Current NJ State Professional Registered Nurse License
• BLS AHA Basic Health Care Life Support HCP Certification
