JOB DESCRIPTION:

• ABEM/AOBEM Board Certificate or Board eligibility letter
• Must have a clean file/background, no malpractice
• Procedure logs (last 2 years)

• Updated CV
• Drivers license
• Color photo
• State medical license (including licensure held in other states)
• CSR
• DEA
• Education certificates
• 5-year malpractice history
• Narrative & legal documentation if provider has claims
• ECFMG if applicable
• ABEM/AOBEM Board Certificate
• Board eligibility letter
• TB/PPD/Quantiferon /Chest x‑ray (within 12 months)
• COVID
• FLU (seasonal)

• All candidates must meet the minimum shift availability listed on the job.
• CV must have all dates in MM/YY to MM/YY format.
• Work history must be in sequential order with most recent on top.
• Any gaps over 30 days must be explained on the CV.
• EPA (please use the attached EPA. It’s also in Trio under your name & then click on Help).
• All licenses, boards, and certs with expiration dates must be listed on CV.
• Anyone with malpractice must provide an NPDB.
• Anyone with legal issues must provide supporting documents.

• First possible date you would be available.
• How many shifts you would be able for per month.
• What month is your schedule completely open.
• Any dates in the next 3-6 months you can’t work.
• Any holidays in the next 3-6 months you would be open to work.
