Description Human Service Specialist (Part-Time) - Community Transition Team

Location: Eau Claire, WI Hours: 16-20 hours per week Schedule: Mon, Wed, Fri 3:45pm-8:00pm; and every other Sat 7:15 AM-11:30 AM Employer: Lutheran Social Services of WI & Upper MI

About the Role

Lutheran Social Services is seeking a Part-Time Human Service Specialist to support our Community Transition Team in the Eau Claire area. Due to contractual requirements,
only female applicants are eligible for this position.

Primary Responsibilities
  • Administer and observe urinalysis, breathalyzer tests, oral fluid swabs, and sweat patch testing
  • Provide services aligned with each client's service plan
  • Participate in recovery teams and provide feedback on client progress
  • Assist with medication monitoring (non-prescriber tasks such as delivery, education, and compliance support)
  • Maintain accurate and timely documentation in accordance with LSS, regulatory, and payer standards
  • Collaborate with clients, service teams, and supervisors to determine effective interventions
  • Maintain client records and make recommendations within scope of practice
  • Attend client meetings, staff development sessions, and clinical supervision
  • Testify in legal proceedings when required
  • Work a flexible schedule based on client needs, including occasional evenings or weekends
  • Perform additional duties as assigned

Qualifications

Required:

  • High school diploma or GED
  • Valid driver's license, reliable transportation, and ability to meet LSS driving/insurance requirements
  • Ability to complete required background, caregiver, medical, and program‑specific checks
  • Ability to read, write, and document services clearly and accurately
  • Basic computer proficiency, including electronic health records and email

Preferred:

  • One (1) year of related work experience

  • Strong interpersonal communication
  • Ability to maintain professionalism in crisis situations, including verbal aggression
  • Ability to work independently and collaboratively
  • Comfort working in community settings, including homes, offices, and outdoor environments
  • Ability to navigate changing software and technology systems
Physical Requirements
  • Frequent bending, stooping, kneeling, and stair climbing
  • Ability to travel daily within the community; occasional overnight travel may be required
  • Moderate noise environment; exposure to outdoor conditions during travel
