Dove Healthcare – West Eau Claire is seeking a Registered Nurse (RN) Case Manager to join our team. If you're ready to take the next step in your nursing career and use your experience to lead, mentor, and support a team while overseeing resident care, this could be the perfect fit.

• Flexible business hours, Monday–Friday
• On-call every 6th weekend and every 6th weekday

• Coordinate and oversee care to ensure quality outcomes for residents
• Support palliative care services focused on comfort, symptom management, and quality of life
• Collaborate with rehabilitation teams to help residents reach their highest level of function
• Supervise day‑to‑day care and team member performance
• Ensure care plans are followed, updated, and compliant with regulations
• Assist with staffing coordination and team support
• Foster a safe, comfortable, and home‑like environment for residents
• Participate in on‑call rotation

• Current Wisconsin RN license
• Graduate of an accredited nursing program
• Previous experience in a related setting
• Strong leadership, organization, and problem‑solving skills
• Ability to manage priorities in a fast‑paced environment
• Team‑oriented with excellent communication skills
