Licensed Practical Nurse, LPN ; Evergreen

The Licensed Practical Nurse will provide general nursing care to patients under the direction of a Registered Nurse and in accordance with physician orders and established protocols. The role integrates company values into daily practice.

• Provides skilled nursing care to patients as ordered by the physician and under the supervision of a Registered Nurse.
• Implements orders for medications and treatments as ordered by the physician and in accordance with the nursing care plan written by the RN.
• Provides information to the Registered Nurse to assist in the development of an individualized plan of care for each patient.
• Participates in evaluation of the effectiveness of interventions in relation to patient responses and seeks guidance in redirection when necessary.
• Reports to supervisor on patient care rendered, problems concerning patients’ needs, and resolution to those needs.
• Helps identify patient problems and potential risks and contributes to the development of interventions.
• Assists with identification of patients’ learning needs and reinforces self‑care and other techniques.
• Appropriately designates applicable charges to individual patients using the hospital’s established system.
• May be required to work during inclement weather and other staffing emergencies.
• Provides an environment conducive to safety for patients, visitors, and staff.
• Assesses the risks for safety and implements appropriate precautions.
• Complies with approved safety and infection prevention standards.
• Maintains regular attendance in keeping with assigned work schedule.
• Completes required trainings, as assigned.
• Performs other duties as assigned to support overall effectiveness of the organization.

• Two years’ experience in inpatient hospital setting; inpatient rehabilitation experience preferred.

• Current state license as a Licensed Practical Nurse.
• Current BLS certification.

• Knowledge of current nursing theory and practice and infection prevention standards.
• Knowledge of accreditation standards (TJC and CMS) and adherence to them.
• Knowledge of clinical operations and procedures and use of proper policies, procedures, and guidelines in patient care.
• General computer skills (data entry, word processing, email, records management).
• Critical thinking skills.
• Effective organizational and time‑management skills.
• Effective written and verbal communication skills.
• Ability to prioritize, meet deadlines, and complete complex tasks.
• Ability to maintain quality, safety, and/or infection prevention standards.

Over the course of a shift: significant amount of standing, walking, bending, reaching, lifting, pushing, and pulling, often for prolonged periods of time. Requires lifting/exerting up to 50 lbs. Possible exposure to bodily fluids. Requires visual acuity for patient assessment and documentation. Requires acute hearing for accurate patient assessment. Requires sufficient manual dexterity to operate equipment and computer keyboard. Requires close vision and ability to adjust focus.
