Sequoia Integrative Medical Services LLC is seeking a compassionate, motivated, and patient-focused Family Nurse Practitioner to join our growing healthcare team serving the Onalaska, La Crosse, and La Crescent communities.

This role provides the opportunity to deliver high-quality primary care services in geriatric and long-term care settings, where you'll build meaningful relationships with patients and make a lasting impact on their health and quality of life.

• Conduct comprehensive patient assessments, physical examinations, and routine follow-up visits.
• Diagnose, treat, and manage acute and chronic medical conditions, including diabetes, hypertension, COPD, asthma, and other common illnesses.
• Complete admission history and physical examinations.
• Perform medication reconciliation during admissions, consultations, and discharges.
• Develop individualized treatment plans focused on prevention, wellness, and chronic disease management.
• Educate patients, families, and caregivers on treatment plans and healthy lifestyle practices.
• Collaborate with physicians, nursing staff, therapists, and interdisciplinary care teams to ensure coordinated, high-quality care.
• Support patients' behavioral health needs through compassionate, whole-person care.
• Maintain accurate and timely clinical documentation in accordance with regulatory and organizational standards.

• Master's or Doctorate degree in Nursing from an accredited program.
• Current Family Nurse Practitioner certification.
• Active Wisconsin Nurse Practitioner license (or ability to obtain prior to start date).
• DEA registration (or ability to obtain prior to start date).
• Excellent clinical judgment, communication, and interpersonal skills.
• Experience in primary care, geriatrics, skilled nursing, or long-term care is preferred but not required.
• New graduates with strong clinical training and a passion for geriatric care are encouraged to apply.
